value of quality medical care and we pay the vast majority of this cost.
Dental Our dental coverage encourages preventative care and makes provisions for
most of the dental services you and your covered family members may need.
Vision Our vision plan helps pay the cost for glasses and contact lenses, as well
as periodic vision exams. The plan features a national network of providers.
Short-Term and Long-Term Disability Inform Diagnostics provides full-time teammates with short and long-term
disability income benefits. These benefits are provided at no cost to you.
Employee Assistance Program (EAP) Our EAP provides access to assistance with a wide range of issues and concerns
that affect your life at home and in the workplace.
Basic Life/Accidental Death & Dismemberment (AD&D) and Optional
Life/AD&D Inform Diagnostics provides full-time teammates with group life and
AD&D insurance at no cost to the teammate. Teammates will receive
two (2) times their basic annual salary, up to a maximum of $300,000 in
coverage. In addition, you may elect supplemental coverage for yourself,
spouse and child.
Health Spending Account (HSA) An HSA is a personal bank account that takes advantage of tax savings
to assist you with out-of-pocket healthcare expenses.
Flexible Spending Account (FSA) An FSA gives you an important tax advantage that can help you pay dental,
vision, and dependent care expenses on a pre-tax basis.
401(k) Inform Diagnostics is committed to providing teammates with resources
to help attain their retirement goals. We will match 100% of the first
3% an employee contributes, then 50 cents on the dollar for the next 2%,
up to the IRS maximum. In addition, all Inform Diagnostics 401(k) contributions
are always 100% vested.
Legal Plan Teammates have access to optional legal plan coverage that helps address
most common legal needs.
Tuition Assistance Educational assistance is available for accredited courses of an established
degree plan (Associates, Bachelors, or Masters Degree) based on eligibility
and business unit approval.
