Request for Missing Information
Submit Requested Information to Inform Diagnostics
Has your office received a Request for Missing Information from the Billing
team at Inform Diagnostics? We use these requests to gather information
to aid us in processing patient claims with payors. We may need information
about the patient’s demographics, insurance, symptoms, diagnosis,
or complaints. Additional clinical information may be required so that
Inform Diagnostics may bill insurance payors for services performed.
For example, the pathologist’s interpretation may conclude “normal”
or non-diagnostic findings. Because International Classification of Diseases
(ICD) codes do not specify “normal” or non-diagnostic findings,
in these cases Inform Diagnostics bills pathology services using the information
from the referring provider—the reason for specimen submission—per
guidance provided by ICD and the Centers for Medicare & Medicaid Services (CMS).
Please continue to provide information such as “rule-out …”,
“suspicious for …”, “favors …”,
and “… vs. …”, as applicable, as this is very
helpful in directing examination and treatment. If your office is contacted
by Inform Diagnostics for additional clinical information, please understand
that we are doing so because ICD and CMS do not allow translation of the
aforementioned examples of terminology into ICD codes, and we rely on
this information for billing purposes.
What to Do When You Receive a Request for Missing Information
When you receive a Request for Missing Information, please complete the
form and return it to Inform Diagnostics:
